- Product name (brand or active ingredient)
- Lot/batch number and expiry date (if available)
- Date and time the event started
- Details of the side effect or adverse event (symptoms, severity, outcome)
- Any other medicines, vaccines or treatments taken at the same time
- Healthcare provider details (if applicable)
- Your contact details (so we can follow up if needed)
